What You Should Bring To The Appointment

You must bring the following documents to streamline your Buckhead Primary and Urgent Care Clinic check-in process.

• A valid photo ID.
• Insurance card information.
  Please note that you do not need an insurance plan to visit the Buckhead Primary Care Clinic. We have reasonable fees for patients without insurance, and we encourage you to contact us for a cost estimate.
• Medical forms.
  New patients will need to fill out a medical form on the day of their appointment. Please fill out the New Patient Form in advance, as this will decrease your wait time on the day of your visit.
